COVID-19 Patient Form

To our current patients, we thank you for being so accepting of our changes in protocol. We continue to book our appointments with social distancing etiquette, as your health is our top priority.

We also ask that only patients enter the office, and that you call us from the parking lot to let us know you are here. Taking your temperature and rinsing with a diluted hydrogen peroxide solution is still part of our protocol.

If you have traveled out of the area and are required to quarantine, please do so for the health of all.

New patients who are children will want to be accompanied by a parent or the parent’s significant other, so we welcome both to your first appointment and will do everything to make you feel safe and comfortable.

We look forward to celebrating the day when most of us have been vaccinated and we can breathe a sigh of relief. Until then, thank you for your patience.

P.S. Parking in the lot has not changed, but if you park in the street, you must do so farther down the street, or park across the street. Thanks.
